Request Appointment

​At our patient portal you can check your laboratory results, communicate more efficiently with our staff and request appointments. If you do not have a user ID and password, please contact us at  (276) 467-2201 to request one. However; your account cannot be activated until you provide us with a valid email address and have read, signed and returned the consent form for the patient portal.

Clinch River Health Services is committed to taking care of you, our patient. We hope to provide you with the best possible care in a warm and friendly environment. We have added a few pieces of information that will benefit you in understanding the care you will receive while a patient at Clinch River Health Services facility. Our goal is to help you feel better and get back to your regular activities as quickly as possible. 
Scheduling an Appointment
  • Most appointments are made for the day or within the week of your call. Please call early if you need a same day appointment.
  • If you are unable to keep your appointment, please call Clinch River Health Services to cancel or reschedule the appointment.  This will allow us to see another patient in your place.
  • Arrive on time for your appointment.  Call to notify Clinch River Health Services, inc. if you are going to be late.

All patients have rights and responsibilities that ensure that you receive the best health care.

Planning Your Visit

Whether you are a long time patient or haven’t been to see us in a while, the following is information that will help you plan your visit to Clinch River Health Services, Inc. and make your appointment go smoothly

A few reminders….
  • Bring all of your medications with you to every appointment.
  • If you have insurance, bring your insurance card. Clinch River Health Services accepts most private insurance, Medicare, and Medicaid insurances.
  • If you cannot keep an appointment, please call Clinch River Health Services to cancel or reschedule the appointment.  This will allow us to see another patient in your place.     
For Your Safety
  • All Clinch River Health Services, Inc. locations are smoke-free.  No smoking is allowed on the property, inside or outside the building.
  • Children in waiting rooms must be watched by parents or guardians.  We are not responsible for your child’s safety.
  • No weapons are allowed in any Clinch River Health Services, Inc. location.

Patient Centered Medical Home (PCMH) Every year, Clinch River Health Services submits evidence to the National Committee for Quality Assurance (NCQA) demonstrating our health center’s ongoing practice of patient-centered care.

What is a Patient-Centered Medical Home (PCMH)? PCMH focuses on you as a whole person and uses a relationship-based approach to your health care.  At CRHS we work on partnering with you to provide the best care for you, recognizing that each patient has unique needs, culture, values, and preferences.

Patient Centered Care Works Toward:

  • Enhancing communication internally and externally
  • Improving patient satisfaction
  • Providing care management
  • Engaging in continuous quality improvement
What you can expect from your Medical Home:
  • Your team can answer questions and help you better understand your health
  • If you need help from other doctors or resources, your team will provide the support you need
  • If you are sick, we will try our best to see you on the day you call
  • When you have concerns about your health, the team will work with you to find the best way to deal with them
  • We offer a sliding fee discount program for medical care
  • We offer onsite assistance for marketplace enrollment and resources for obtaining insurance coverage
How does my Medical Home work?
  • You have unique health needs.  In your Medical Home, we care about you as a whole person
  • Your Medical Home team can answer your questions to help you better understand your health care needs
  • Your team can answer questions in person, by phone or through our Patient Portal.  We also provide after-hours support
  • If you need help from other doctors or resources, your team will be there to provide the support you need
  • Your Medical Home will work best if you provide us with your medical history
  • Your team will track your progress to show you how your medical care is working
  • Your Medical Home will provide you with high quality health care including up-to-date educational resources
